For Order #156261139, in DEMO061033, the order details give a few comments why the working state sell LMT order could not be canceled or modified:

I contacted support to inquire what these comments mean. They hadn't seen these comments before, felt that the demo servers were having an issue, and force-cancelled the set of sell LMT orders on that contract.

I'm writing this bug report to give devs a headsup so they can make a fix if possible.

Hello, Chris.

Thank you for reporting.

Order #156261139 was filled 06/14/15 before your Cancel command #156269263.
But during this time we had issues with our DEMO servers and Fill was lost.
